Lutron Grafik Eye QS Wireless 6-zone Lighting Controller - QSGRJ-6P-TWH. Just removed from a fully functional home, it's in 100% perfect working order and looks to me to be essentially perfect cosmetically as well. (see pics to make your own assessment). Can't see a single thing wrong with it in any way.
